Report Lost or Stolen Cell Phone

Overview

Use this form to report a lost or stolen County cell phone.

Who can submit this request?

  • Any Pasco BOCC Employee assigned a County cell phone

How we process your request

Typical resolution time:

1-2 business days

Process:

After you submit your request, IT will:

  • Review the request
  • Attempt to locate the device remotely
  • Lock/Remote Wipe/Suspend Line of service on Lost or Stolen cell phone
  • Send a closeout notification upon resolution

Before you submit a ticket, you will need to:

  • Provide the Last Date Used and Last Seen Location
  • Determine if the device cannot be located if you want the line of service terminated from the carrier and a new device ordered? (New phone number)
Note: If a new phone line is ordered, a chargeback account number will be required.
