Overview
The purpose of this form is to report issues regarding County issued cell phones, e.g., cracked screen, apps not opening, will not power on, etc.

Who can submit this request?
How we process your request
Typical resolution time:
1-3 business days (depending on severity of issue)
Process:
After you submit your request, IT will:
- Review the request
- Contact you for further information, if necessary
- Examine/diagnose the problem, and troubleshoot until resolution
- Send a closeout notification upon resolution
Before you submit a ticket, you will need to:
- Provide the phone number of the affected phone
- Proivde an alternative means of contact if the phone is not functioning
Note: When describing the issue, please try to be as specific as possible, so we can resolve your issue in a timely manner.
