The purpose of this form is to request a new alarm line.
Note: If the site is using cellular service, then an alarm line is not needed.

How we process your request
Typical resolution time:
3-5 business days
Process:
After you submit your request, IT will:
- Review the request and assign it to the purchasing technician
- Line of service is requested
- Alarm Line put in service
- Send a closeout notification upon resolution
Before you submit a ticket, you will need to:
- Make note of the location of the new alarm line with building address
- Notate which department will be paying for the service
