Overview
The purpose of this form is to request a review of data intended for use within a software application that incorporates AI. The requestor should be familiar with the data classification categories applicable to the data that will be made available to the AI system.
Data Classification Examples
PII (Personally Identifiable Information)
Data that can identify a specific individual. Examples:
- Full name
- Home address
- Email address
- Phone number
- Social Security number
- Driver’s license / ID numbers
- Date of birth
- Passport number
- Biometric identifiers
- Financial account numbers
- Medical information (when linked to identity)
PCI (Payment Card Information)
Sensitive payment data regulated by PCI-DSS. Examples:
- Credit card number
- Debit card number
- Cardholder name
- Expiration date
- CVV/CVC security code
- Authentication data (PIN, encrypted PIN blocks, etc.)
Exempt / Non‑Sensitive / Public Information
Information that does not pose a privacy or security risk and is allowed to be publicly accessible. Examples:
- Publicly available business contact info
- Job titles
- Company addresses
- General product information
- Public website content
- Aggregated or anonymized data
- Non‑identifiable statistical data

Any Pasco BOCC Employee with organization management approval.
